Relocation Service Coordination
Relocation service coordination aims to help a person who resides in certain institutions (Nursing Facilities, nursing facility, regional treatment center, hospital or ICF/DD) to gain access to medical, social, educational, financial, housing, and other services and supports that are necessary to move to the community setting.
Our team of Housing Coordinators will help the person develop and implement a relocation plan. We will coordinate and monitor the plan to support the individual in moving to the most integrated setting. We will assist the person in identifying and overcoming barriers and support the housing search process. Upon finding a home, we will assist with coordinating the move.

Moving Home Minnesota Services
Moving Home Minnesota is a federal demonstration project that aims are moving individuals residing in eligible facilities (Nursing Facilities, nursing facilities, regional treatment centers, hospitals, or ICF/DD) by providing access to medical, educational, financial, and other supports necessary to move back into the community setting.
Our team of Housing Coordinators will help the person develop and implement a relocation plan. We will coordinate and monitor the plan to support the individual in moving to the most integrated setting. We will assist the person in identifying and overcoming barriers and support the housing search process. Upon finding a home, we will assist with coordinating the move.

Available Services
Under Moving Home Minnesota there are a wide range of different services depending on needs of the recipient of services. At Redeemer, we are able to provide the following services under Moving Home Minnesota:
- Transition, Planning, Transition Coordination, and Services: Help the person develop and implement a relocation plan. Coordinate and monitor the plan to support the individual in moving to the most integrated setting.
- Demonstration Case Management: Assist with access to and navigation of social, health, educational, vocational, and other community and natural supports and services based on a person’s values, strengths, goals and needs.
- Comprehensive Community Support Services: Help the individuals connect with available supports and services. Monitor and maintain ongoing tenancy support including dispute resolution, budget, scheduling, and appointments.
