Housing Stabilization Services
Housing Stabilization Services is a Medical Assistance program that aims to help individuals with disabilities, including mental illness and substance use disorder, and seniors find and maintain housing. Finding and keeping stable affordable housing is important to the health of individuals.
The purpose of these services is to support an individual’s transition into housing. It promotes increasing long-term stability in housing in the community and avoids future periods of homelessness or institutionalization.
Housing Stabilization Services has three stages:
- Consultation Stage – Our team will help eligible individuals who do not have waiver or targeted case management services create a person-centered housing plan. We will support the person to select their housing transition or sustaining provider.
- Transition Stage – Our team will help eligible individuals find a home of their own. We will assist the person in identifying and overcoming barriers and support with the housing search process. Upon finding a home, we will assist with coordinating the move. Our team will advocate for the individuals we work with to help the person overcome their barriers and find a new home.
- Sustaining Stage – Our team is dedicated to helping individuals maintain housing and prevent homelessness. We may support the person with budgeting, paying rent, and identifying community resources. Our goal is to support the person with maintaining long-term housing stability.
